The AMAC iCare Desktop™ is the central repository for patient information and responses transmitted daily from Health Buddy® and PERS Buddy®. The iCare Desktop provides a universal platform for multiple providers to deliver coordinated disease management monitoring.
The iCare Desktop allows providers to enroll patients, assign standard or customized disease dialogues and enables healthcare providers to access and monitor pivotal aspects of a patient's health and well-being from any Internet-ready computer.
Patient responses to symptom, behavior and self-care knowledge questions, via their Health Buddy® or
PERSBuddy® appliance, are analyzed and risk-stratified by color: HIGH (red), MODERATE (yellow), LOW (green). This easy-to-read patient snapshot enables care mangers to closely monitor each patient's disease progression and proactively intervene before a condition becomes acute and escalates into a costly medical episode.
The AMAC iCare Desktop™ allows multiple users, with security clearance, to access data through a secure HIPAA compliant data center. Care managers can enroll patients, assign disease specific programs, establish patient specific health parameters and generate compliance and trend analysis reports.
